• Referral - Inclusion Services are requested by parent/caregiver during registration or recommended by the Program Supervisor.
• Parent/Caregiver Interview - The Inclusion Services Coordinator will conduct an initial interview with the parent/caregiver to determine the participant’s preferences, interests, needs and strengths and collect any additional supporting documentation as needed (IEP, ISP, etc.).
• Create Unique Support Plan - The Inclusion Services Coordinator will create an Environmental Support Plan and/or Behavior Support Plan.
  • The Environmental Support Plan is intended to proactively create a successful setting by outlining accommodations to the environment, activity set up and delivery of instructions.
  • The Behavior Support Plan is intended to outline potential challenging behaviors, as well as steps staff, can take to prevent, minimize and respond to challenging behaviors while encouraging and acknowledging positive behaviors.
• Initial Staff Training - Staff members will be trained on the participant's support plan.
• Ongoing Observations and Training - The inclusion Services Coordinator will provide ongoing observations to ensure that the Support Plan meets the needs of the participant. Staff training will be provided as needed based on these observations.
• Ongoing Communication - Staff will complete Participant Observation Forms if a challenging behavior occurs and Participant Progress Notes regularly. These forms will be reviewed by the Inclusion Services Coordinator and Program Supervisors in order to communicate the participant’s progress with parents/ caregivers.
• Support Plan Revision - Support plan revisions will be made as necessary based on the need of the participant.
